Se analizaron 465 certificados; 45.6% corresponden a defunciones fetales y 25% a muertes post-neonatales. Las muertes perinatales representaron 67.8%; 52.6% ocurrieron en niños menores de 2,500 g de peso al nacer. Las malformaciones congénitas se presentan como la primera causa de muerte con 22.1%. Las infecciones fueron la segunda causa con 17.3%. Los problemas de placenta y cordón representan 12.5%. En 24.6% de los certificados analizados no se pudo establecer la causa de muerte. La aplicación de la metodología GEMAS MALITAS para asignar las causas básicas de muerte feto-infantil permite disminuir la causa indeterminada de muerte, sobre todo en los casos donde no se puede hacer autopsia anatomopatológica de los fetos recién nacidos e infantes. A review of death certificates was made between the 1st of July and the 31st of December of 1999 to assign the basic cause of death using the methodology developed by our Research Center (CEMIYA); 465 death certificates were analyzed; 45.6% were fetal deaths and 25% were post-neonatal deaths. Perinatal deaths were 67.8%. Low birth weight babies less than 2,500 were 52.6%. Congenital malformations were the leading cause of death with 22.1%. Infections were the second cause with 17.3%. Cord and placenta problems accounted for 12.5%. In 24.6% of the certificates it was impossible to establish the cause of death. The application of the methodology GEMAS MALITAS to assign basic causes in fetal infantile deaths is useful to avoid the term “indeterminate,” as a cause of death when the autopsy of the fetus or infant cannot be made.Publicación Acceso abierto Programa Madre Canguro : primeros resultados de una cohorte de niños seguidos desde la unidad neonatal hasta la semana 40 de edad postconcepcional.(2013-09-24) Torres, Javier; Palencia, Diana; Sánchez, Diana Margarita; García, Jorge; Rey, Humberto; Ecandia, Carlos ArmandoIntroducción: El bajo peso al nacer es un problema de salud pública en los países en vía de desarrollo, que en América Latina produce una mortalidad neonatal 35 veces mayor a la esperada. En la unidad de recién nacidos del Hospital Universitario del Valle, 75% de los neonatos tienen <2,500 g. Objetivos: Evaluar los niños seguidos desde la unidad neonatal hasta las 40 semanas de edad gestacional en el programa madre canguro. Materiales y métodos: Se evaluó la cohorte de recién nacidos que ingresaron al programa madre canguro en la unidad de recién nacidos del Hospital Universitario del Valle, Cali, Colombia, con peso <2,001 g, que habían superado los principales problemas de adaptación neonatal y que contaran con una familia dispuesta a colaborar. Se excluyeron aquellos neonatos con malformaciones mayores o letales tempranas como encefalopatía hipóxica isquémica, hipertensión pulmonar, hemorragia intraventricular grado III y IV, y niños abandonados o a los que estaban en adopción. Se recogieron datos sobre crecimiento físico, duración de la lactancia materna, morbilidad, mortalidad, reingresos, fármacos, estancia y giro cama desde el egreso hasta las 40 semanas de edad gestacional. Resultados: Un total de 66 niños alcanzaron el término, 40 (60.6%) niñas, y 26 (39.4%) niños, con promedios (los rangos se dan entre paréntesis) de los siguientes datos al nacer: 32 semanas (27-38), edad gestacional; 1,434 g (700-1,950), peso; 41 cm (35-47), talla; y 28 cm (22-41), perímetro cefálico, respectivamente. Al egreso los datos correspondientes fueron: 34 semanas (31-39) edad gestacional; 19 días (4-48) edad postconcepción; y 1,552 g (1,359-2,239), peso. Durante el seguimiento tuvieron cinco consultas en promedio y una ganancia de peso total promedio de 18 g/kg/día. Al egreso 96% recibían lactancia materna y a las 40 semanas 94%, que fue exclusiva en 67% y en 51%, respectivamente. La anemia y la enfermedad por reflujo gastroesofágico fueron las principales causas de consulta por urgencias. Fue necesario rehospitalizar 3 (4.5%) niños, dos para crioterapia y uno por apnea, ninguno murió. Conclusiones: Hasta el cumplimiento de las 40 semanas de edad gestacional, el programa mostró ser una alternativa segura para el manejo de niños con bajo peso al nacer, pues les garantiza un egreso temprano, contacto piel a piel, un crecimiento adecuado y una alimentación inicial basada en leche materna. Introduction: Low birth weight is a health problem in non-developed countries, with a neonatal mortality rate 35 times higher in Latin America than the expected one. At the neonatal care unit of the University Hospital in Cali, Colombia, 75% of all live newborn infants are <2,500 g. Objectives: To evaluate the infants who were followed up until 40 weeks of postconceptional age in the kangaroo mother program. Methodology: A cohort study was performed in newborns infants <2,001 g who were admitted to the neonatal unit of the Hospital Universitario del Valle, Cali, Colombia, if they had overcome all major adaption problems to extrauterine life and had a family willing to follow the instructions. Exclusion criteria were lethal or major malformations, early detected major conditions arising from perinatal problem (severe hypoxicischaemic encephalopaty, pulmonary hypertension and intracerebral hemorrage III or IV), and either abandoned children or those who were given for adoption. All patients were monitored in physical growth, length of breast-feeding, morbility, mortality, readmissions, hospital stay and bed turn. Measurement were made at discharge, at 40 weeks of postconceptional age. Results: A total of 66 infants reached term, 40 (61%) were girls and 26 (39%) were boys, with a mean birth gestational age of 32 weeks (27-38) and mean birth weight of 1,434 g (700-1,950). Infants were discharged with mean gestational age of 34 weeks (31-39), mean postconceptional age of 19 day (4-48) and mean weight of 1,552 g (1,359-2,239). During the following they had a mean weightgain of 18 g/kg/day. A large proportion of infants received breastfeeding: 96% at discharge and 94% at term. Exclusive breastfeeding: 67% at discharge and 51% at term. Anemia and gastroesophageal reflux disease after discharge were the main causes of morbidity. Only three (4.5%) infants were readmissed before the 40 week, two for criotherapy and one for apnea, none died. The hospital stay at neonatal unit was 17.5 days and the bed turn was 1.37 patients/bed/month. Conclusions: Kangaroo mother program showed to be a safe alternative to the care of low birth weight infants, since it gives at least an early discharge, skin-to-skin contact and a good quality nutrition based initially on breast-feeding.
