Publicación Acceso abierto Malaria : eficacia de amodiaquina-sulfadoxina-pirimetamina, estado nutricional y variantes alélicas del gen CYP2C8.(2012-04-23) Carmona Fonseca, Jaime; Guzmán Pérez, Valentina; Maestre Buitrago, AmandaProblema: La respuesta terapéutica antimalárica depende de múltiples determinantes asociados con el plasmodio (especie, mutaciones, cantidad, etc.) y al hospedero (nutrición, genes, metabolismo, etc.), pero los últimos son poco conocidos. Objetivos: Evaluar en pacientes con malaria falciparum no complicada, tratados con amodiaquina-sulfadoxina-pirimetamina (AQ-SP), algunas relaciones entre la respuesta terapéutica (RTA), el estado nutricional y las variaciones alélicas del gen CYP2C8. Metodología: Estudio clínico controlado, con asignación aleatoria, balanceado, no ciego. La RTA se evaluó según la Organización Mundial de la Salud. Se hizo análisis antropométrico, se midieron las concentraciones plasmáticas de retinol, ferritina y selenio; se analizaron las variantes 2C8*1 (silvestre), 2C8*2 (Il29F) y 2C8*3 (R139K y K399R) del gen CYP2C8. Resultados: Se evaluaron 33 pacientes, todos con respuesta terapéutica adecuada con AQ-SP; 10% presentó deficiencia de retinol, 25% de selenio y 40% de ferritina. Sólo un paciente presentó la variante CYP2C8*2 en forma heterozigótica y el resto fueron homocigóticos para el alelo silvestre de este gen. Ninguno presentó la mutación R139K en CYP2C8*3. Del alelo K339R de CYP2C8*3 no se pudieron obtener fragmentos aptos de digerir, aún haciendo adaptaciones del método y no fue posible conocer la razón de este hecho. Estos datos concuerdan con los resultados de otro análisis similar en 23 pacientes, tratados solo con amodiaquina: 22% presentaron alguna variante (5 con CYP2C8*2 y 2 con CYP2C8*3). En el gen CYP2C8*3 se identificó sólo la mutación R139K, presente en 2 individuos. Conclusión: Sólo uno de los 33 pacientes (3%) presentó la variante CYP2C8*2, en forma heterozigótica; el resto fueron homocigóticos para el alelo silvestre de esta variante. Ninguno presentó la mutación R139K de la variante CYP2C8*3. Es el primer informe para Latinoamérica. Problem: Therapeutic response to antimalarials depends on multiple determinants associated with the parasite (species, mutations, parasitaemia, etc.) and the host (nutrition, genes, metabolism, etc.), but little is known about the host factors. Objectives: To evaluate in non-complicated falciparum malaria patients undergoing treatment with amodiaquinesulfadoxine- pyrimethamine (AQ-SP), some relationships between treatment response, nutritional status and characteristics of the gen CYP2C8. Methodology: A randomly assigned, balanced, non blind, controlled clinical design. Treatment response was assessed according to WHO 1998 criteria. Analysis included anthropometry, plasma levels of retinol, ferritin and selenium, and assessment of 2C8*1 (wild), 2C8*2 (Il29F) and 2C8*3 variants of CYP2C8 (R139K y K399R). Results: 33 patients were studied, all of them evidenced adequate treatment response, 10% had retinol deficiency, 25% selenium deficiency and 40% low ferritine levels. One patient exhibited the variant Il29F of CYP2C8*2 in a heterozygous fashion, the remaining individuals were homozygous for the wild form of this gene. The mutant R139K of CYP2C8*3 was absent in all individuals. Amplification fragments obtained of K339R (CYP2C8*3 gen) were not suitable for digestion, regardless of the modifications performed. These results confirm previous findings made in 22% of 23 patients in whom some variation was observed (5 in CYP2C8*2 and 2 in CYP2C8*3). For CYP2C8*3 the mutant R139K, was observed in 2 individuals. Conclusion: only one of the 33 patients (3%) had CYP2C8*2 in a heterozygous fashion, the remaining were homozygous for the wild allele of this variant. None of the patients had the mutation R139K of the CYP2C8*3 variant. This is a novel report for Latin America.Publicación Acceso abierto Malaria vivax en niños: aspectos clínicos y respuesta a la cloroquina.(2012-04-23) Carmona Fonseca, Jaime; Uscátegui, Rosa M.; Correa, Adriana MaríaIntroducción: En Colombia, no hay información actualizada sobre malaria vivax en niños. Objetivo: Describir el cuadro clínico-paraclínico de la enfermedad y evaluar la eficacia de la cloroquina (dosis estándar) para curar el ataque agudo de paludismo vivax en niños (4-10 años). Metodología: Diseño experimental, balanceado, no ciego; 82 pacientes, residentes en Turbo o El Bagre. Seguimiento, 30 días. Resultados: Los síntomas y signos concordaron con la literatura. Hubo desnutrición de largo plazo en 62%, anemia en 53%, retinol bajo (19 μg/dl), recuento leucocitario normal, pruebas hepáticas y creatinina normales. A los 25-28 días postratamiento todas las alteraciones, excepto la desnutrición, habían desaparecido. Falla de la cloroquina: dos posibles fallas tardías. Proporción de falla según tres técnicas de análisis según la intención de tratar: 2.4% (0 a 24%), según el protocolo 2.6% (0 a 25%), según el peor escenario 7.3% (0 a 29%). Conclusión: El cuadro clínico-paraclínico fue similar al de los adultos. El paludismo, más que la desnutrición, fue responsable de las alteraciones clínicas-paraclínicas. La cloroquina sola, sin primaquina, resultó altamente eficaz para el ataque agudo de malaria vivax en niños y debe conservarse como primera opción terapéutica. Introduction: In Colombia is not very common to find updated information about vivax malaria in children. Aims: Describe the clinical and paraclinical disease picture and evaluate the standard dose of chloroquine effectiveness as a cure for the acute attack of vivax malaria in children between 4 and 10 years old. Methods: Experimental design, balanced, not blind, 82 patients and habitants in El Bagre and Turbo, Colombia. Followup: 30 days. Results: Symptoms-signs agreed with the known. There were found 62% long-term malnutrition, 53% anemia, low retinol (19 μg/dl), normal leukocyte count, normal liver tests and normal creatinine coefficient. After 25-28 day of treatment, all alterations had disappeared on children except malnutrition. According with the analysis techniques, the chloroquine late failure proportion was: by the intention to treat 2.4% (0 to 24%), by the protocol 2.6% (0 to 25%), and in the worst-case scenario 7.3% (0 to 29%). Conclusion: The clinical and paraclinical depict was similar than adults. Malaria, was the main responsible for clinics and paraclinical alterations. Chloroquine, without primaquine, proved highly effective for the acute attack of vivax malaria in children and should be retained as the first therapeutic option.Publicación Acceso abierto Relationship between Duffy blood groups genotypes and malaria infection in different ethnic groups of Choco- Colombia.(2012-11-20) Gonzalez, Lina; Vega, Jorge; Ramirez, Jose Luis; Bedoya, Gabriel; Carmona Fonseca, Jaime; Maestre, AmandaBackground: The negative homozygous condition for the Duffy blood group (Fy-/Fy-) confers natural resistance to Plasmodium vivax infection. In this direction, studies carried out in Colombia are scarce Objective: To describe the relationship between Duffy genotypes in three ethnic communities in La Italia (Chocó) and malaria infection. Methodology: a descriptive, cross-sectional study in symptomatic and asymptomatic malaria subjects. Sample size : Afro American, 73; Amerindian (Emberá), 74 and Mestizo, 171. Presence of Plasmodium infection was assessed by thick smear and the status of the Duffy gene by PCR and RFLP in order to identify the substitutions T-46C y A131G which origin the genotypes T/T, T/C , C/C y G/G, G/A, A/A. Results: Infection by Plasmodium was detected in 17% with 62% due to P. falciparum and 27% to P. vivax. Duffy genotypes were significantly associated to ethnicity (p=0,003). Individuals with the C/C, A/A diplotype were exclusively infected by P. falciparum, whereas other diplotypes were infected with either species. In the Amerindian and Mestizo populations, the frequency of the T-46 allele was 0,90-1,00, among Afrocolombians this was 0,50, equal to the C allele and with absence of heterozygous At locus 131, the highest frequency of the G allele was 0,30 in Amerindians and the A allele was 0,69 in Afrocolombians. Conclusions: In the Amerindian and mestizo populations studied, a predominance of the allele T-46 (FY+) was observed, but P. vivax was not the most common. Infection by P. vivax was out ruled in all FY- individuals. realizados en Colombia son escasos. Objetivo: describir la relación entre los genotipos Duffy en tres comunidades étnicas de La Italia (Chocó), y la malaria. Metodología: estudio descriptivo, transversal con individuos sintomáticos o asintomáticos de malaria. Tamaño muestral por etnia: afrocolombiana 73; amerindia 74, mestiza171. La infección plasmodial se estudió por gota gruesa y el estado del gen Duffy por PCR y RFLP con el fin de identificar los cambios T-46C y A131G que originan los genotipos T/T, T/C , C/C y G/G, G/A, A/A. Resultados: se encontró infección plasmodial en 17% con 62% por P. falciparum y 27% por P. vivax. Se halló relación entre genotipos Duffy y etnia (p=0,003). Los individuos con diplotipos C/C,A/A se infectaron exclusivamente con P. falciparum, mientras que otros diplotipos por las dos especies. En amerindios y mestizos la frecuencia del alelo T-46 fué 0,90-1,00, en afrocolombianos fué 0,50, al igual que el alelo C, con ausencia de heterocigotos. En el locus 131 la máxima frecuencia del alelo G fue 0,30 en amerindios y la máxima del alelo A fue 0,69 en afrocolombianos. Conclusiones: En las poblaciones amerindia y mestiza estudiadas predomina el alelo T-46 (FY+), pero no predomina en ellas la infección por P. vivax. Ningún individuo FY- tuvo infección por P. vivax.Publicación Acceso abierto Seguridad hepática y hemática de la amodiaquina en el tratamiento de la malaria no complicada por Plasmodium falciparum.(2013-07-12) Pineros, Juan Gabriel; Lopez, Mary Luz; Carmona Fonseca, Jaime; Blair, SilviaIntroducción: Actualmente existe un número limitado de antimaláricos eficaces, entre ellos amodiaquina; sin embargo, su uso se ha restringido por informes previos de toxicidad hepática y hemática a dosis superiores a 1,500 mg administradas como profiláctico para malaria. No obstante, en dosis terapéuticas antimaláricas los efectos adversos son de intensidad leve o moderada, e incluyen náuseas, vómito y prurito. Objetivo: Evaluar la toxicidad hepática y hemática de la amodiaquina en dosis y tiempo establecidos para tratar la malaria por Plasmodium falciparum no complicada. Metodología: Diseño longitudinal con determinación no ciega del efecto. Se captaron 57 pacientes, seguidos por 10 días (evaluación clínico-parasitológica). Resultados: Antes del tratamiento, las variables hemáticas y hepáticas mostraron alteración leve y se normalizaron postratamiento, que fue 100% eficaz. Los días 5 y 10 del tratamiento todas las variables estaban normales, lo que sugiere ausencia de efectos tóxicos imputables al medicamento. Los efectos adversos fueron pocos, leves y desaparecieron completamente el día 10. Conclusiones: Usada en la dosis (25 mg/kg peso) y el tiempo (3 días) definidos para el tratamiento de la malaria por P. falciparum sin complicaciones, la amodiaquina no mostró efectos adversos ni toxicidad hepática ni hemática. Background: At present there are few effective antimalarial drugs, amodiaquine is one of them; however, its use has been restricted by previous information about hematic and hepatic toxicity when it is administered as prophylactic at doses greater than 1,500 mg. But at therapeutic doses, the side effects are either slight or of moderate intensity and include nausea, vomit and pruritus. Objective: To evaluate the hepatic and hematic toxicity of amodiaquine administered at doses and time recommended for treatment of uncomplicated Plasmodium falciparum malaria. Methods: Longitudinal design with no blind determination of the effect. A total of 57 patients were included and followed up for 10 days (clinical-parasitological evaluation). Results: Hematic and hepatic variables showed slight alteration previous treatment and were normal postreatment. Therapeutic efficacy of amodiaquine was 100%. All variables were normal at days 5 and 10, suggesting absence of toxic effects imputable to amodiaquine. The side effects were few, slight and disappeared completely at day 10. Conclusions: Amodiaquine administered at doses (25 mg/kg weight) and time (3 days) established for treatment of uncomplicated Plasmodium falciparum malaria is safe, it did not show neither hematic nor hepatic toxicity.Publicación Acceso abierto Seguridad hepática y hemática de la sulfadoxina-pirimetamina en el tratamiento de malaria por Plasmodium falciparum no complicada.(2012-12-05) Carmona Fonseca, Jaime; López, Mary Luz; Piñeros, Juan GabrielIntroducción: Entre los fármacos ampliamente usados en la actualidad para el tratamiento de la malaria por Plasmodium falciparum se encuentra la sulfadoxina-pirimetamina, que es comúnmente administrada en combinación con otros medicamentos. Los eventos adversos informados se han asociado con altas dosis, uso como profiláctico o en personas con hipersensibilidad a las sulfas. Objetivo: Evaluar la toxicidad hepática y hemática de la sulfadoxina-pirimetamina (SP) administrada en dosis y tiempos terapéuticos para la malaria por P. falciparum no complicada. Metodología:Como parte de un estudio con diseño experimental, con determinación abierta del efecto, se hizo evaluación hemática y hepática a 17 personas tratadas con SP. En Turbo (Antioquia, Colombia), los pacientes se captaron consecutivamente y se siguieron por 10 días. Resultados: Las variables hepáticas y hemáticas estuvieron alteradas durante el cuadro malárico antes del tratamiento y se normalizaron al poco tiempo del inicio del mismo (1-5 días), que fue eficaz en 100% de los pacientes (medido el día 10). La normalidad de todas las variables en los días 5 y 10 (fase postratamiento) sugiere la ausencia de efectos tóxicos imputables al medicamento. Los efectos adversos fueron pocos y leves y desaparecieron en el control del día 10. Conclusiones: La SP no mostró eventos adversos, toxicidad hepática ni hemática usada en la dosis y el tiempo definidos para el tratamiento de la malaria por P. falciparum sin complicaciones. Introduction: Sulfadoxine-pyrimethamine is an antimalarial used currently in worldwide for non-complicated falciparum malaria. This drug is administrated in combination with other ones. Previously adverse events had been reported with high doses, used in malaria prophylaxis and patients with hypersensibility to sulfas. Objetive:To evaluate hepatic and haematic toxicity of treatment with sulfadoxine-pyrimethamine (SP) in non-complicated falciparum malaria. Methodology: This was a non-blinded experimental design. In Turbo (Antioquia, Colombia), 17 subjects treated with SP were evaluated for liver and hematic function. All individual were followed for 10 days. Results: Before treatment, liver and hematic function tests were slight altedered. Hematic and liver variables returned to physiological levels after treatment. Treatment had 100% efficacy. All tests were within normal levels throughout the following period (postreatment); this suggests absence of toxic effects associates with treatment. Adverse effects were few and slight, and disappeared on day-10. Conclusions: When is used in time and dose for treatment of non-complicated falciparum malaria, SP neither increased adverse events nor hepatic or hematic toxicity.Publicación Acceso abierto Status of allele frequency and diversity of Plasmodium falciparum msp1, msp2 and glurp before implementation of an artemisinin-based combined therapy in Northwestern Colombia.(2014-02-19) Maestre, Amanda; Arango, Eliana; Carmona Fonseca, JaimeIntroduction. The status of msp1, msp2 and glurp allele frequency and the diversity of Plasmodium falciparum in Northwestern Colombia before the implementation of an artemisinin-combined therapy have been explored only by a few authors and in a relatively small number of samples from this highly endemic region. Objective. To evaluate the frequency of msp1, msp2, and glurp alleles and the diversity of P. falciparum in two Colombian regions before the use of an artemisinin-combined therapy. Methods. This study was part of a major anti-malarial efficacy trial designed as a random, clinically-controlled study for which 224 subjects were recruited. Region 2 of msp1 and msp2 (central region) were amplified by a nested PCR; glurp (region R2) was amplified by a semi-nested PCR. Results. For msp1, five genotypes were observed, representing the K1, MAD20, and RO33 allelic families. All samples corresponded to a MAD20 150 bp allele. For msp2 (IC family), two alleles were detected and for glurp, eight were observed. A total 33 haplotypes were detected. Conclusions. Analysis of glurp can be used to successfully genotype parasite populations in the new studies in Colombia aimed at exploring Plasmodium spp. population dynamics. In addition, analysis of msp1 and msp2 can also be of value for comparisons with past studies, but not when the objective is to study parasites obtained from the same patient in a reduced period of time; for instance, during treatment efficacy studies. Introducción. La frecuencia alélica y la diversidad de msp1, msp2 y glurp de Plasmodium falciparum en el Noroccidente de Colombia antes de la introducción de la terapia combinada con artemisinina, han sido exploradas por pocos autores y en números relativamente bajos de muestras clínicas de esta región de alta endemicidad. Objetivo. Evaluar la frecuencia alélica y la diversidad de msp1, msp2 y glurp de P. falciparum en dos regiones colombianas antes del uso de la terapia combinada con artemisinina. Métodos. El estudio fue parte de un ensayo mayor sobre la eficacia de la terapia antimalárica con un diseño clínico aleatorio controlado, en el cual 224 sujetos fueron reclutados. La región 2 de msp1 y msp2 (región central) se amplificaron por medio de una PCR anidada; glurp (región RII) fue amplificada con una PCR semianidada. Resultados. En msp1, se observaron 5 genotipos que representaron las familias alélicas K1, MAD20, y RO33. Todas las muestras correspondieron al alelo 150 bp de MAD20. En msp2 (familia IC), se detectaron 2 alelos y en glurp, se observaron 8. Un total de 33 haplotipos fueron detectados. Conclusiones. El análisis de glurp puede ser utilizado para genotipificar exitosamente poblaciones parasitarias en nuevos estudios en Colombia con el objeto de explorar la dinámica de las poblaciones de Plasmodium spp. Adicionalmente, el análisis de msp1 y msp2 puede también ser de valor para comparar con estudios pasados, pero no cuando el objetivo es estudiar parasitos obtenidos de un mismo paciente en un periodo reducido de tiempo, por ejemplo, durante estudios de eficacia terapéutica.Publicación Acceso abierto The effects of Plasmodium vivax gestational malaria on the clinical and immune status of pregnant women in Northwestern Colombia.(2013-11-22) Yasnot, María Fernanda; Perkins, Douglas Jay; Corredor, Mauricio; Yanow, Stephanie; Carmona Fonseca, Jaime; Maestre, AmandaObjetive: The study explored the effects of Plasmodium vivax infection on the balance of pro- versus anti- inflammatory cytokines and chemokines and their relationship with some clinical and epidemiology outcomes. Methods: Thirty-five pregnant women were recruited. Of these, 15 subjects had malaria at delivery (GM+), and 20 had no exposition to infection throughout the pregnancy (GM-) and at delivery. Epidemiological and clinical data were recorded after reviewing the clinical records. At delivery, whole blood from the mother as well as placental tissue was collected. Diagnosis of infection was performed by thick smear and a polymerase chain reaction (PCR). Expression of pro-inflammatory and anti-inflammatory cytokines and chemokines was measured by a real time PCR. Results: The clinical and epidemiological variables explored were similar in both groups, with the exception of gestational age. When comparing the GM+ group with the GM- group, it is clear that although the differences generally are not significant, pro- inflammatory cytokines are elevated in both maternal blood and placental; anti-inflammatory ones are elevated in the mother and reduced in the placenta, and the chemokines are reduced in both compartments, except for MCP-1 which is elevated in all. Conclusion: The results appear to be strongly affected by the small number of women with GM by P. vivax at childbirth. Additional studies are needed with larger groups in this and other regions of the country. Objetivo: En este estudio se determinó el efecto de la infección por Plasmodium vivax en el balance de citoquinas pro-inflamatorias/anti-inflamatorias y quemoquinas y su relación con algunas variables epidemiológicas y clínicas. Métodos: Se reclutaron 35 gestantes, 15 con malaria en el momento del parto (GM+) y 20 sin malaria en ningún momento de la gestación (GM-) Los datos epidemiológicos y clínicos fueron colectados a partir de la historia clínica. En el momento del parto fueron tomadas muestras de sangre periférica materna y tejido placentario. El diagnóstico fue realizado mediante gota gruesa y reaccion en cadena de la polimerasa (PCR). La expresión de citoquinas pro-inflamatorias/anti-inflamatorias y quimioquinas, fueron medidas por PCR en tiempo real. La expresión de citoquinas pro-inflamatorias/anti-inflamatorias y quemoquinas, fueron medidas por PCR en tiempo real.. Resultados: En las variables epidemiológicas y clínicas, los datos fueron similares en ambos grupos. Al comparar el grupo GM+ con el grupo GM-, resulta claro que, aunque las diferencias, en general, no son significativas, las citoquinas proinflamatorias están elevadas tanto en sangre materna como placentaria, las antiinflamatorias están elevadas en la madre y reducidas en la placenta, y las quimioquinas están reducidas en ambos compartimentos, excepto la MCP-1 que está elevada en ambos. Conclusión. Los resultados parecen estar fuertemente afectados por la cantidad pequeña de mujeres con MG por P. vivax en el parto. Es necesario adelantar estudios adicionales con más mujeres tanto en esta región como en otros lugares.
