Background: The Instituto Municipal de Salud de Pereira, reported in 2003, an increase of level in tuberculosis (TB). This increase was hypothetically related with AIDS in Pereira. Methods: All of the patients with TB in public hospitals were examined with Elisa test and Western Blot test. This research lasted 5 months. Since patients’ records were incomplete it was necessary visiting patients’ houses in order to obtain more data. Results: Were reported 94 cases of TB disease in Pereira in 5 months; among them 58.5% were males. Median age was 41 years old. Pulmonary TB was present in 90.4% patients; of these 80.9% were highly infectious, with acid-fast bacilli (AFB) identified on sputum smear. Only 7 patients were reactive to the Elisa test, and of them 3 patients were confirmed with Western Blot test. The prevalence of co infection was 3.2% IC95% 0.7, 9.04. Of the three patients two were women and one had tuberculous meningitis. Conclusions: The prevalence of TB and HIV co infection is low if compared with others cities in Colombia. The TB increase wasn’t related to co-infection with HIV. It’s very important to have a quality control of patients’ records. Introduction: In Colombia, there are few studies that have evaluated the health-related quality of life (HRQOL) of individuals infected with HIV/AIDS through MOSSF-36, WHOQOL-BREF or WHOQOL-HIV-BREF and no studies have been reported comparing some of its psychometric properties. Objective: To describe the HRQOL and to analyze the psychometric properties of two generic and one specific instruments to measure HRQOL in a group of people living with HIV/AIDS in Medellin, Colombia, 2009. Methodology: Cross-sectional study in a nonrandom sample of 187 subjects with HIV/AIDS. The MOSSF-36, WHOQOL-BREF, and WHOQOL-HIV-BREF were applied. The description was made with proportions and summary measures; reliability was assessed with Cronbach’s alpha, Pearson’s correlation coefficients were used for internal consistency and discriminant validity, and the convergent/ divergent validity of the concepts of HRQOL with the correlation among the domains of the instruments. Results: Higher HRQOL scores were observed in Physical Function, Environment, Level of Independence and Spirituality. The MOSS-36 showed an excellent level of reliability, internal consistency and discriminant validity in its eight domains; the WHOQOL-BREF presented excellent psychometric properties in Physical Health, Environment and Social Relations, and good properties in Psychological Health; whereas, the WHOQOL-HIV-BREF had excellent psychometric properties in Social Relations, Environment, Spirituality and Independence and good properties in Physical and Psychological Health. There were correlations above 0.4 among domains of different instruments, which show the convergence of the measurement of HRQOL with the instruments applied. Conclusion: The MOSSF-36 showed a perfect profile of psychometric characteristics, the WHOQOL-BREF and WHOQOL-HIV-BREF were an acceptable level and the constructs of the three instruments converge, indicating good development of tools for studying HRQOL in HIV/AIDS infected individuals in Colombia.Publicación Acceso abierto Characterization of treatment failure in HIV positive patients in the Colombian Caribbean region.(2015-02-10) De La Hoz, Juan Manuel; Bolaño, Laura; Cárdenas, Oriana; González, Robertulio; Sabbag, José; Palacio, Lucy; Alonso, Luz Marina; San Juan Vergara, Homero; Cervantes Acosta, GuillermoBackground: Treatment failure in patients receiving antiretroviral therapy against human immunodeficiency virus (HIV) is always a concern. Objective: To examine the correlates associated with treatment failure in patients living in the Colombian Caribbean city of Barranquilla, an aspect that was poorly studied in this region. Methods: Treatment failure (TF) was evaluated in a cross-sectional study from virological, immunological and clinical standpoints. Results: It was established that 29.5% of patients under highly active antiretroviral therapy (ART) could be considered in TF. Among those, virological failure was most frequent (20.9%), followed by immunological- (14.0%) and clinical failure (4.7%). In patients showing lack of adherence to the treatment, the likelihood of suffering from treatment- and virogical-failure were respectively increased by 6.67-fold and 12.19-fold, compared with patients showing good adherence. Although there was no statistically significant association, TF tended to be more frequent in young adults, in patients with low income and, low level of education. When ART regimens were compared, there was no apparent difference in TF between regimens based on non-nucleoside reverse transcriptase inhibitors and those based on protease inhibitors. This is very important in the context of recent ART strategies, such as early-initiated ART, aimed at achieving longterm infection control. Conclusions: Is confirmed the importance of treatment adherence to avoid TF and further highlights the importance of educating HIV-infected patients in all parts of the world, especially those individuals with a lower socio-economic status. Publicación Acceso abierto Expression and activation of intracellular receptors TLR7, TLR8 and TLR9 in peripheral blood monocytes from HIV-infected patients.(2013-07-09) Valencia Pacheco, Guillermo; Pinzón Herrera, Francisco J.; Cruz López, Juan J.; Vera Gamboa, Ligia del Carmen; Pavía Ruiz, Norma; Santos Rivero, Adrián; Sánchez Lugo, Saulo; Puerto, FernandoIntroduction. TLR´s play a role in host defense in HIV infection recognizing the viral DNA or RNA. Their activation induces a signaling pathway that includes the proteins MyD88, IRAK4, TRAF6 and the transcription factor NF-kBp65. Objective. To determine the expression of TLR7, TLR8 and TLR9, and activation of its signaling pathway in monocytes from patients infected with HIV. Methods. Expression of TLR7, TLR8 and TLR9 was determined in monocytes from HIV-infected patients (n = 13) and control subjects (n = 13), which were activated with specific ligands. The expression of MyD88 and NF-kBp65 were determined by flow cytometry; IRAK4 and TRAF6 were studied by immunoblotting. Results. No statistical difference was found in the expression of TLR7, 8 and 9 in monocytes from patients compared to controls, but we observed the non-significant increased expression of TLR9 in patients. The activation showed no significant difference in the expression of MyD88 and NF-kBp65 in patients when compared to controls, but were decreased in stimulated cells over non-stimulated cells. IRAK4 and TRAF6 were not detected. Conclusions. No statistical difference was observed in the expression of intracellular TLRs, MyD88 and NFkBp65 in monocytes from patients compared to controls. This was probably due to effective antiretroviral therapy being received at the time of study entry. Additional studies are needed (ARTV) under controlled conditions that include infected patients with and without ARVT, responders and non- responders, and work with different cell populations. Publicación Acceso abierto HIV and risk behaviors of persons of low socio-economic status, Popayan-Colombia (2008-2009).(2013-07-09) Mueses, Hector Fabio; Pinzón, María Virginia; Tello, Inés Constanza; Rincón Hoyos, Hernan Gilberto; Galindo, JaimeObjective: To determine HIV presence and risk behaviors of persons of low socio-economic status in the city of Popayan- Colombia. Methods: Cross-sectional study; between 2008 and 2009, 363 participants of Popayan signed informed consent and received pre and post HIV test counseling. Socio-demographic characteristics and history of STDs, risk behaviors and previous HIV testing were assessed. Descriptive statistics, correlations and multivariate logistic regression were calculated Results: Mean age 33,5±10,2; 66%women. Frequency of HIVpositive patients was 3.86% (95% CI: 1.87-5.85), greater in men (7.38%; p=0,013). Greater frequency of HIV-positive patients was observed in people age 29-37, those without a stable partner, and those with history of risky alcohol consumption (more than five drinks in 2 hours) . Conclusions: HIV-positive patients frequency in this population was greater than national estimate for general population, aged 15-49 in Colombia, with even greater frequency in men. This study suggests that characteristics associated with low socioeconomic status, in economically active population, without a stable partner and with risky alcohol use, can potentially increase risk of HIV infection. Do differences exist among neighborhoods?.(2013-07-09) Girón Vargas, Sandra Lorena; Palacio Cochéz, Hannia Stella; Mateus Solarte, Julio CésarIntroduction: HIV/AIDS is a global health priority. About 40% of new infections occur among heterosexual youth by means of sexual contact. In Cali, comune 13, 15 and 20 account for 11.5% of the prevalent cases and 18.0 % of incident cases. Objective: To establish any differences in risk behaviors for HIV among young people 15-24 yrs of age from two areas of Cali, Colombia. Methods: We carried out a cross-sectional study among young people between 15 and 24 years of age in these comunes. The selection was done with a two-stage probability sampling. We estimated the prevalence of sexual relationships without condom usage, sex with multiple partners, and sex under the effects of alcohol and through logistical regression we identified the related factors. Results: In comunes 13, 15 and 20, the prevalence of unprotected sexual relationships in the last 12 months and the prevalence of sex with two or more partners was 70%; and 38% of young people had sex under the effects of alcohol. In both areas, the intention was positively related to the risk behaviors. We found socio-demographic factors, intentions, and beliefs that increase the opportunity to display these behaviors. The effect of these factors differs by comune. Conclusions: We observed a high prevalence of risk behaviors for HIV related to socio-demographic factors, intentions and beliefs that warrant interventions appropriate for local realities. Natural killer T cells with an invariant T-cell receptor and restricted by CD1d (iNKT) are a subgroup of lymphocytes with a very strong immunoregulatory potential; their quick response and their ability to produce Th1 and Th2 cytokines are determinant factors that influence the development of innate and adaptive immune responses. The physiological role of iNKT cells has been well documented in anti-tumor immune responses, the development of tolerance in immune-privileged organs and the control of autoimmune diseases. Despite the fact that the immunoregulatory potential of these cells has been well documented, less is known regarding their role in the immune response against infectious agents, in particular to human pathogenic viruses. This paper synthesizes the search in the most important biomedical data bases (Pubmed, Medline, OVID), including studies on the phenotypic characterization of these cells and functional studies that evaluated their role in the interaction between hosts and viruses. iNKT cells have a heterogeneous participation during the anti-viral immune responses, depending on the type of virus; indeed, in some instances the iNKT-cell responses can be involved in the tissue damage associated to the anti-viral responses. In humans, iNKT cells are apparently essential for an effective immune response against Varicella Zoster virus, while it is still controversial their role in the control of other viral infections. Studies in animal models have shown the first evidences on the therapeutic potential of this lymphocyte subpopulation. Aim: To determine the prevalence and factors related with the presence of oral manifestations in HIV/AIDS patients who attended health institutions in Cali-Colombia. Materials and methods: A cross-sectional descriptive study was made in 319 patients who attended four health institutions in the city of Cali. The inclusion criteria were: being HIV positive patients, being over 18 years old, attending medical checkups and accepting participation. Pregnant women and inmates were excluded. Results: The general prevalence of oral manifestations was 77.1% (CI 95%: 72.4-81.7). The most frequent oral manifestation was xerostomia with 41.1% (CI 95%: 35.5-46.6), followed by the manifestations from mycotic origin [(35.7% (CI 95% 30.4-41)] bacterial [34.2% (CI 95%: 28.9-39.4)] virals [16.6 (CI 95% 12.5-20.7)], oral ulcers [7.5 (CI 95%: 4.6- 10.4)] and neoplastics [3.8 (CI95% 1.6-5.8)]. The population studied did not show non-Hodgkin’s lymphoma. In the multivariate analysis, the presence of oral injuries was associated with time of HIV/AIDS diagnosis during the previous 12 months, detectable viral load, and presence of removable dental prosthesis and self-report of dry mouth. Conclusions: The prevalence of oral manifestations registered in this population is high compared with those reported world-wide. Considering the association found, our results suggest that oral injuries are initial clinic characteristics of the disease and may be useful to predict treatment failure and progression from HIV infection to AIDS.Publicación Acceso abierto Quality of life in persons living with HIV–AIDS in three healthcare institutions of Cali, Colombia.(2011-11-16) Patricia Valencia, Claudia; Eugenia Canaval, Gladys; Marín, Diana; Portillo, Carmen J.Antecedents: The Human Immunodeficiency Virus is currently considered a chronic disease; hence, quality of life is an important goal for those suffering the disease or living with someone afflicted by the virus. Objectives: We sought to measure the quality of life in individuals living with acquired immunodeficiency syndrome virus and establish its relationship with socio-demographic and clinical variables. Methods: This is a cross-sectional, descriptive study with a sample of 137 HIV-infected individuals attending three healthcare institutions in the city of Cali, Colombia. Quality of life was measured via the HIV/AIDS-Targeted Quality of Life (HAT-QoL) instrument. The descriptive analyses included mean and standard deviation calculations. To determine the candidate variables, we used the student t test and the Pearson correlation. The response variable in the multiple linear regression was the score for quality of life. Results: Some 27% of the sample were women and 3% were transgender; the mean age of the sample was 35 + 10.2 years; 88% had some type of health insurance; 27% had been diagnosed with AIDS, and 64% were taking antiretroviral medications at the time of the study. Quality of life was measured through a standard scale with scores from 0 to 100. Participants’ global quality of life mean was 59 + 17.8. The quality-of-life dimensions with the highest scores were sexual function, satisfaction with the healthcare provider, and satisfaction with life. The highest quality-of-life scores were obtained by participants who received antiretroviral therapy, had health insurance, lower symptoms of depression, low frequency and intensity of symptoms, and no prior reports of sexual abuse. Eight variables explained 53% of the variability of the global quality of life. Conclusions: Those receiving antiretroviral therapy and who report fewer symptoms best perceived their quality of life. Implications for practice: Healthcare providers, especially nursing professional face a challenge in caring to alleviate symptoms and contribute to improving the quality of life of their patients.
