dc.description.abstractLa prevalencia de alergia alimentaria (AA), es del 10 al 15%; y la de intolerancia a los alimentos, del 85 al 90%. En lactantes, la causa más común de AA, es la alergia a la proteína de la leche entera de vaca (APLEdeV). Hay mayor riesgo de presentar APLEdeV ante la presencia de antecedentes de atopia o exposición temprana a las proteínas de la leche entera de vaca (PLEdeV). El inicio de los síntomas está asociado a la ingesta del alergeno, en este caso a la PLEdeV, bien sea por leche materna en una mamá que se encuentre lactando y en su alimentación ingiera PLEdeV o por ingesta de fórmulas infantiles o directamente PLEdeV. Se propone un diagnóstico y manejo unificado e interdisciplinario entre el dermatólogo, el alergólogo y el gastroenterólogo pediatra. El diagnóstico de la APLEdeV, es dependiente de la severidad de las manifestaciones clínicas, del tiempo de evolución y de los factores de riesgo asociados: el estándar de oro, son las pruebas orales de reto doble ciego placebo controladas. El manejo incluye una adecuada relación médico-paciente-padres, dieta de eliminación en la madre lactante, continuidad en la leche materna, recomendaciones del inicio de la ablactación, en algunas ocasiones fórmulas infantiles especiales, y excepcionalmente, medicamentos. En general, el pronóstico de los niños con APLEdeV es muy bueno. SUMMARY Allergy to the protein of the whole cow’s milk The prevalence of food allergy (FA), is around 10- 15%; and of food intolerance, 85-90%. In infants, the FA cause commonest, is the allergy to the protein of the whole cow’s milk (APWCM). There is greater risk of presenting antecedents of atopic or early exhibition to proteins of the whole milk of cow (PWCM). The beginning of the symptoms is associate to the ingestion of the alergen, in this case to the PWCM, or is by maternal milk in a mother who is breastfeeding and in her feeding she directly ingests PWCM or by ingestion of infantile formulas or PWCM. One sets out a diagnosis and management unified and interdisciplinary between the dermatologist, the allergologist and the gastroenterologist pediatric. The diagnosis of the APWCM, is depending of the severity of the clinical manifestations, the time of evolution and the factors of risk associate: the gold standard, placebo is the oral tests of double challenge blind controlled. The treatment includes a suitable relation doctor-patient-parents, diet of elimination in the nursing mother, continuity in maternal milk, recommendations of the beginning of the new foods, in some occasions special infantile formulas, and exceptionally, drugs. In general, the prognosis of the children with APWCM is very good.spa
