| dc.description.abstract | Las caídas causan dependencia e inmovilidad en los ancianos. El objetivo de este estudio fue comparar las caídas y sus problemas relacionados en tres grupos: ancianos con ejercicio regular, sedentarios, e institucionalizados. Se tomaron al azar 135 ancianos independientes en su marcha y se dividieron en tres grupos de 45 cada uno. Se hizo un cuestionario donde se registraron todos los incidentes de caídas, casi-caídas, miedo a caer, consecuencias en actividades básicas cotidianas (ABC), y autopercepción en salud. De la muestra total 45.9% tuvieron por lo menos una caída en el último año, promedio, 1.11 (DE 1.99) en ancianos institucionalizados; 1.26 (DE 1.52) en sedentarios; y 0.73 (DE 1.23) en activos; no hubo diferencias significativas. El sentirse saludable se relaciona con la presencia de caídas (p = 0.029). En 34.8% hubo por lo menos una casi-caída, con diferencia estadística entre institucionalizados y activos (p <0.05%). Los ancianos con autopercepción de salud buena o muy buena fueron 64.4% en activos vs 20% en institucionalizados (p <0.05). La restricción total en ABC por caídas o casi-caídas fue: física, 25.8%; instrumental, 30.6%; y social, 38.7%. En 62.9% había miedo a caer, y 26% de ellos tenían ABC limitadas con diferencias significantes entre los grupos (p <0.01). Son notorios los ancianos que tienen caídas, casi-caídas, miedo a caer y restricciones en las ABC. El ejercicio puede ser una estrategia buena para aumentar la autopercepción de la salud y para prevenir el descenso en las ABC debido al miedo a caer. Falls cause dependence and immobility in older people. The objective of this study was to compare falls and their related problems in three groups: regular exercisers, sedentary and institutionalized elders. A total of 135 independent in gait elders were randomized and divided into 3 groups, 45 people each one. A questionnaire was made. All falls, near-falls, fear of falling, outcomes in ADL, and self-perceived health were registered. Of the total sample 45.9% had at least one fall in the last year, the mean was 1.11 (SD 1.99) in institutionalized, 1.26 (SD 1.52) in sedentary and 0.73 (SD 1.23) in regular exercisers, without statistical differences. Elders perceived as healthy were related to falls (p = 0.029). Of them 34.8% had at least one near-fall with statistical differences among exercisers and institutionalized (p <0.05). People with self-perceived health either well and/or very well were 64.4% in exercisers vs 20% in institutionalized elders (p <0.05). Total restriction in ADL was: physical 25.8%, instrumental 30.6%, and social 38.7%; 62.9% had fear of falling and 26% of them had limited ADL with significant differences among groups (p <0.01). Elders who had falls, near-falls, fear of falling and ADL restriction are remarkable. Exercise may be a good strategy to increase self-perceived health and to prevent decreasing ADL from fear of falling. | spa |
