Publicación Acceso abierto Importancia de la nutrición enteral en niños con Fibrosis Quística.(2015-11-24) Jiménez, RafaelLa fibrosis quística (FQ) o mucoviscidosis, se describió originalmente en 1938 por Anderson, siendo la más frecuente y de mayor letalidad de los trastornos genéticos en algunas poblaciones. Su carácter multisistémico hace que el enfoque de su tratamiento sea muy complicado. Los niños con FQ tienen una elevada mortalidad y morbilidad debido a tres factores fundamentales: El primero de ellos es la insuficiencia respiratoria como consecuencias de la colonización de las vías áreas; el segundo se deben a la malabsorción o maldigestión de los nutrientes producto fundamentalmente de la insuficiencia pancreática asociada con la enfermedad; y el tercero es consecuencia de los dos anteriores, una desnutrición que puede abarcar todas las formas clínicas descritas y que consecuentemente empeora tanto las funciones respiratorias como digestivas. El tener una estrategia nutricional adecuada en relación con las cantidades de nutrientes, la calidad de los mismos, la vía y la forma en que se van a usar, resulta de vital importancia en los resultados de la terapéutica del niño con FQ, siendo la vía enteral la vía de elección porque permite el uso de nutrientes de elevada calidad que respondan a las necesidades incrementadas en el niño. El presente informe discutirá la importancia de la nutrición haciendo énfasis en la evaluación de una estrategia adecuada donde se involucran la familia y el equipo de salud, así como la importancia de una adecuada evaluación de la conducta alimentaría y de la alimentación enteral en el manejo nutricional del niño con FQ. SUMMARY Cystic fibrosis or mucoviscidosis was described initially in 1938 by Anderson, is the most frequent and lethal genetic disorder among some populations. Its systemic character does the treatment very complicated. Children’s with CF have a increased rate of mortality and morbidity owing to three main factors: The first of them is the respiratory failure because the colonization of the airway, the second of the factors is due to the nutrients malabsorption or maldigested as a consequence of the pancreatic failure associated to the disease and the last of factors is a consequence of the former factors, a malnutrition with all the possible described clinic manifestation of this entity that do worse both the respiratory and digestive functions. To have an appropriate nutrition strategy in relation with nutriments, amounts and quality, way and how will be use, have a great importance on the CF therapeutic, being the enteral nutrition the main way because allows the use of nutriments with high quality that respond to increased energy necessities in the children with CF. The present report will discuss the importance of nutrition doing emphasis in the assessment of an adequate strategy which involves both the family and the health team, as well as, the importance of the appropriate feeding behaviour and the enteral nutrition in the nutritional treatment of the children with CF.Publicación Acceso abierto Nutrición Parenteral En El Neonato(2015-11-24) Durán Bravo, Pedro N.; Pérez Vásquez, MaríaRESUMEN La nutrición parenteral (NP) se refiere a nutrientes provistos por diferentes rutas del tracto gastrointestinal, en general a través del sistema circulatorio. Se utiliza en neonatos como parte del tratamiento médico integral y se considera un método apropiado para administrar nutrientes en esta población de alto riesgo. La NP neonatal se ciñe estrictamente a los principios fundamentales de la nutrición humana (debe ser completa, equilibrada, suficiente y adecuada) para utilizarse como parte del tratamiento del neonato enfermo, para asegurar la transición adecuada del proceso de crecimiento del período prenatal al postnatal, con una ganancia de peso similar a la tasa de ganancia de peso intrauterina sin sobrecarga metabólica, para mantener o recuperar el estado nutricional y reducir la morbilidad y mortalidad asociada con la desnutrición. Administrar agua y electrolitos en el neonato pretérmino se hace en tres etapas: transición, estabilización y crecimiento; indicada como parte del tratamiento integral en todos los casos que no puedan ser nutridos por vía gastrointestinal total o parcial. Se inicia en forma lenta y progresiva desde el primer día de vida, controlando si el aumento que se hace es tolerado. Contraindicada en acidosis metabólica severa y/o retención de CO2 mientras se corrige, en choque, en hiperbilirrubinemia severa, insuficiencia renal aguda, diátesis hemorrágica severa e hiperglicemia severa mientras ésta se corrige. La suspensión del soporte nutricional se hace de manera gradual, disminuyendo el volumen y la concentración de la mezcla, teniendo en cuenta que se cubran los requerimientos nutricionales por vía enteral. ABSTRACT The parenteral nutrition (NP) talks about to nutrients provided by routes different from tract gastrointestinal, in general through the circulatory system. It is used in newborn as it leaves from integral the medical treatment and is considered at the moment a method appropriate to administer nutrients in this population of high risk. The neonatal NP must be fitted strictly to the principles fundamental of the human nutrition (it must complete, be balanced, sufficient and be adapted) to be used like part of the treatment of the newborn patient, to assure the adapted transition the process of growth of the prenatal period to the postbirthday, with a gain of weight similar to the rate of intrauterine gain of weight without metabolic overload, to maintain or to recover the nutritional state and to reduce associated morbi-mortality to the undernourishment. When administering water and electrolytes in the newborn preterm we do it in three stages: transition, stabilization and growth. Indicated like part of the integral treatment in all the cases that cannot be nourished by total gastrointestinal route or partially. It is possible to be initiated in progressive form from the first day of life. To initiate slowly, controlling if the increase that becomes has been tolerated. Contraindicated in severe metabolic acidosis and/or CO2 retention while it is corrected, severe shock, hyperbilirrubim, acute renal insufficiency, severe hemorrage and high glucose while this is corrected. The suspension of the nutritional support must be made gradually, diminishing the volume and the concentration of the mixture, considering that cover the nutritionales requirements by enteral route.
